Job Description

Position: Home Care Package, Care Manager Location: Clayton - Carer's Network Head Office Salary: $75,000 - $85,000 per annum + car + superannuation About us: Carer's Network is committed to providing a supportive and collaborative work environment where every team member is valued and empowered to succeed. About the role: Carer's Network is seeking an experienced Care Manager to support our team and look after a caseload of participants in our Home Care Package and Support at Home Program. If you\'re passionate about providing exceptional care and have the experience to match, we want to hear from you! Experience Required: Demonstrated experience in a Care Management role, preferably within the Aged Care Home Care Sector. Strong understanding of home care and support at home packages and familiarity with government funding mechanisms. Proven track record of working to company organisational objectives and goals. Experience in providing guidance, support and mentoring colleagues. Excellent communication skills with the ability to interact effectively with clients, families, caregivers, and healthcare professionals to promote and empower consumer participation and choice through the provision of effective and results-based case management to our Home Care Package clients. Demonstrated ability in problem-solving, conflict management and decision-making, especially in fast-paced environments. Good working knowledge of the Aged Care Quality Framework and Home Care Standards. A compassionate approach to care, driven by a genuine desire to improve the lives of others. Ability to work autonomously and in a team. Responsibilities: Lead dedicated professionals to deliver high-quality home care services. Oversee the allocation, and monitoring of Home Care and Support at Home packages to ensure our participants’ needs are met effectively. With your personable personality, you will maintain strong relationships with existing clients, their families, caregivers, and external stakeholders. Coordinate with internal departments to streamline processes, optimise service delivery and maintain budgets. Ensure compliance with regulatory requirements and industry standards. Implement strategies to continuously improve service quality and client satisfaction. Qualifications & Experience: Degree or Diploma in Community Services (desirable) Certificate III or IV in Aged Care, Disability and Individual Support (desirable).
