Experienced RN Case Manager needed for acute care case management and discharge planning in a hospital setting. This is not a behavioral health role. The RN will manage a case load of up to 25 patients per shift, coordinating interdisciplinary care and discharge services, while also ensuring compliance with CMS, The Joint Commission, and hospital policies.

Key Responsibilities:

• Handle admission reviews, utilization management, discharge planning, and care coordination
• Develop and manage treatment plans and facilitate family meetings
• Manage prior authorizations and DME/SNF/ambulance arrangements independently
• Coordinate care for diverse patient populations including neonates, adults, and geriatrics
• Use EPIC charting system for documentation
• Collaborate with PT, social workers, and other interdisciplinary support teams
• Weekend rotation required (minimum 4 weekend shifts in 6-week period) Ax
