Job Details

• At least 1 year of hospital based diagnostic x-ray experience and surgery training
• May need to float onsite within skill set or to sites within 40 miles if needed

Job Requirements

• BLS (Must be AHA)
• ARRT certification
• Wisconsin state license (current license and primary source verification within 30 days prior to start)

Schedule Information

• Call based on department needs - currently at 3rd shift call (2300-0600/2300-0700), 2 days a pay period (1 per week)
• Every other week or every 3rd weekend based on need
• Holidays: rotating with staff

Additional Details

• HIPAA, Joint Commission -NPSG, OSHA Topics training completed within a year of start and updated yearly with minimum 80% score
• Documented proof of two MMR vaccines or Positive IGG Titer required, no declinations accepted
• Fit Test required, preferred prior to start, updated annually
• Documentation of Hepatitis B vaccination series completion or declination signed and dated by traveler
• Influenza Vaccine required during flu season (Oct 1 - Mar 31), exemptions accepted with mask requirement
• Positive IGRA or PPD results with negative chest X-ray within a year of start and annual TB questionnaire
• Statement of good health or completed physical exam prior to first assignment, updated annually
• Driver's License/State ID must be valid and match address requirements
• 2 Step PPD OR IGRA within required timeframes
• Documented proof of Varicella immunity or vaccination, no declinations accepted
• Tdap vaccination valid for 10 years, no declinations accepted
• Ascension Core Competencies checklist/testing completed within a year and updated annually
• Ascension Medical Imaging Contract Worker Onboarding and Education Attestation required
• Ascension Profile, 2 Professional References, Criminal Background Check, EPLS, Skills Checklist, 12 Panel Drug Screen, Education Verification, Confirmation of Assignment, Employment Verification required
