As a Registered Nurse Case Manager , you’ll be the bridge between medical care and human comfort — guiding patients and families through their journey with empathy, skill, and dignity. You’ll collaborate closely with a team that truly values your voice, your expertise, and your heart.
How You’ll Make an ImpactDeliver exceptional, compassionate hospice care that honors each patient’s dignity and wishes
Assess patients’ physical, emotional, and spiritual needs — treating the whole person , not just the illness
Support families with education, comfort, and guidance through every step of their journey
Communicate closely with the interdisciplinary team to ensure seamless, personalized care
Provide comfort and compassionate support during end-of-life transitions
Teach and inspire others by sharing your hospice knowledge and heart for service

Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Bachelor’s)
Current RN license in the State of Michigan
At least 1 year of clinical experience (hospice or home health preferred)
Compassion, flexibility, and a heart for meaningful work
Valid driver’s license, insurance, and reliable transportation (50% travel required)
Ability to lift, move, and support up to 50–100 lbs. as needed
