Responsibilities

• Assist in the development, review & revision of the plan of care.
• Provide nursing care according to physician’s plan of treatment and individual plan of care.
• Observe, record & report reaction to treatment & any changes in the client's condition to appropriate personnel and/or physician.
• Provide ongoing instruction & supervision to the client, family or other care team personnel in the plan of treatment.
• Monitor & record the patient's condition & document provided care services through electronic charting.
• Participate in education programs, quality assessment activities, nurse meetings, & other staff meetings as required.

Requirements

• Active Licensed Practical Nurse or Registered Nurse License
• Problem solving skills & ability to multi-task
• Excellent Communication Skills
• Positive, Professional attitude
