The Mental Health Practitioner Intake and Referral assesses patient’s needs for acute crisis treatment albeit determining the appropriate level of care and referring the patient to the appropriate level of care. This position also provides counseling services including assessment, diagnosis, and treatment for patients experiencing social/behavioral/mental health issues. This position does not directly manage any other caregivers. Essential functions include coordinating screening, admission, assessment, and referral of patients for psychiatric inpatient beds; determining level of care and making treatment recommendations; providing outpatient referrals; developing and coordinating referrals to outside agencies and hospitals; providing insurance pre-certifications and authorizations; communicating with patients, families, referral sources, hospitals, and insurance companies; assisting discharge planning; facilitating multi-disciplinary care plans; participating in patient and family education; performing psychosocial assessments; providing diagnostic, preventative, and therapeutic treatment services; respecting patient confidentiality; functioning as a member of the interdisciplinary care team; providing individual, group, and family therapies using evidence-based modalities; working in a fast-paced environment; completing assessments and treatment plans compliant with regulatory standards; assessing risk and legal status of patients; providing crisis intervention; offering psychoeducation and case management; maintaining knowledge of regulations; coordinating after-care; attending meetings and trainings; supporting a healing environment; documenting patient care; providing therapeutic presence; supporting continuous improvement; maintaining safety; complying with laws and ethics; delivering customer service; and performing additional duties as assigned. Additional duties specific to PAT/PES Unit and Sage View include conducting crisis and risk assessments, daily rounding, treatment planning, and coordinating multidisciplinary team meetings. Education required is PhD, PsyD, or Master’s in Clinical Psychology, Social Work or related field. Licensure required is current unrestricted Oregon licensed clinical psychologist, LCSW, LMFT, or LPC or ability to obtain licensure as Registered Associate within 3 years. Experience required is one year in behavioral health field. Physical requirements and PPE use are specified. Schedule is full-time, Sunday-Wednesday 0700-1730.
